Mission Statement
Welcome to our practice! Dr. Hernandez and his staff strive to provide the best possible medical and surgical care in a personalized and compassionate manner, based on our patient’s individual needs.
What makes us different? At our practice, all diagnostic evaluations, diagnostic procedures, and surgical procedures are performed only by Dr. Hernandez, a fellowship trained, board certified dermatologist, and not by physician assistants or nurse practitioners.
Patient Forms / Patient Portal
To expedite your visit, please complete your new patient forms in the patient portal prior to your arrival. When scheduling your appointment, you will receive an email with information to register in the portal. The link will expire in 72 hours. If it has been more than 72 hours, please contact our office at (941) 764-7773 to reactivate the link. Once in the portal, click on “My Health”, and complete the forms on each of the sections located on the left-hand menu of your screen.
If you prefer to not use the portal, kindly arrive 30 minutes before your appointment to complete your new patient forms at the office.
We will also need you to bring the following:
- Insurance cards.
- Photo identification.
- List of medicines.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.
